The Transition Subgroup was established in July 2024 and is formed of family carers and professionals from charities, education, health, and social care. The group has identified issues faced by young people with learning disabilities in transitioning from children to adult services and co-produced the actions that are needed to ensure young people and their families experience a positive transition.
Recently, the Transition Subgroup has:
- Updated the ‘Transition to Adulthood‘ section of the Co-Produced Action Plan
- Created case study ‘Transition Journeys‘, showing the experiences of young people with learning disabilities before, during, and after transition, and identifying where action could and should have been taken to improve their experiences
- Fed into the development of a report, published in July 2026, that brings together the experiences of young people with learning disabilities and their families, research, and case studies, to make recommendations on actions that should be taken
We will continue to work collaboratively to improve transition to adulthood. We will also look to identify good practice examples to share best practice and encourage the development of improved support.
