Learning Disability Nurse:

Emergency Plan:
- Have a plan detailing who to inform and what steps to take in an emergency.
During a Hospital Stay
Admission:
- Inform the learning disability nurse immediately.
- Prepare your family member using visual guides like Books Beyond Words.
Treatment:
- Communicate clearly with the medical team, especially about symptoms and pain.
- Monitor for “diagnostic overshadowing” where disabilities may obscure health issues.
- Be aware of critical care and DNAR guidelines. Seek a second opinion if needed.
Discharge and Post-Hospital Care
- Ensure a discharge letter is provided for the GP.
- Contact social services if additional home care support is needed.
End-of-Life Support
- Plan and document end-of-life care preferences.
