This is a ‘living document’, capturing what is currently being done, and/or actions that are planned – it will be updated as and when progress is made and as more actions are taken. It brings together a range of activities and work being carried out by various individuals and organisations and aims to co-ordinate and amplify the impact. Improving understanding of the role of Learning Disability Liaison Nurses (also known as Acute Liaison Nurses) to reduce the health inequalities faced by people with a learning disability 

Changing Our Lives has been commissioned by Health Education England to create a competency framework and toolkit setting out the qualities of a good Acute Liaison Nurse, what support they need, and how they can help improve the health of and reduce the health inequalities faced by people with a learning disability.
